Tips for Applicants Applying in Greece

Item 5 - Social Security Number: You must complete this item. Failure to provide a Social Security Number if you have been issued one can result in a $500 fine enforced by the IRS. If you have never had a Social Security Number (SSN), write zeros in the boxes. If you have a number, but do not remember it, call the Embassy's Federal Benefits Unit for assistance. You may contact the Federal Benefits Unit Monday, Wednesday and Friday, 8:30 a.m. to 12:30 p.m. at 210-720-2426 or by e-mail at FBU.Athens@SSA.gov

 

Item 6 – Complete this item so that the Embassy can notify you when your passport is ready.  

 

Item 7 - Mailing Address: Complete this item with a local address, either your home address or the address where you are staying. Please note that if you choose to have your new passport sent to a different address, complete the address on the prepaid envelope or inform the courier when arranging for the delivery.


  
Item 17 (Item 18 on DS-82) - Permanent Address or Residence: Complete this item with any address you consider to be your permanent address. This need not be in Greece or the U.S.



Items 19 (Item 20 on DS-82) - Travel Plans and Emergency Contact: This information is not required, but may be helpful in processing your application and in assisting you in the future.
Oath & Signature: You are required to sign in the presence of the consular officer at this office. You must read the "Acts or Conditions" as outlined in the instruction pages before you sign. If any mentioned acts or conditions have been performed by or apply to the applicant, cross out only the applicable portion(s) and submit an explanatory signed statement with the application.

The form may be completed by typing the required information into the online Passport Application Wizard.  The application should then be downloaded and printed on white A4 or letter-size paper, with each page on a separate sheet of paper.

The instructions on this form as printed both by the Passport Application Wizard and on the electronic application are written for individuals applying for a passport within the United States. As applications are processed differently outside the United States, you should follow instructions for submitting your application to Embassy Athens as outlined on our website.  

 

Complete sections 1 to 22.  If choosing to complete the application form using a pen, it should be completed in block letters.
