Medical Purposes (B1/B2)
In general, travelers not eligible for the Visa Waiver Program seeking admission into the U.S. for medical purposes or wish to accompany someone who will travel for medical purposes require valid B1/B2 visas.

Documents required for medical purposes visa are:
- A valid passport that has been signed by the applicant and, where applicable,
- Previous passport with previously issued visa or
- Police report in case of theft or loss of a previous passport with valid visa. The report may be issued by a local police station or consular office or other foreign authority.
- If you have ever been arrested and /or have a criminal conviction, have a medical ineligibility, or have been denied entry into or deported from the United States, you will be required to furnish documents relating to your situation in support of your application.
- The paid duplicate receipt from Piraeus Bank (see step 3).
- The printed confirmation page with barcode generated by the DS-160 online application form (see step 4).
- One photograph that meets the required specifications (see step 5). Even if you have successfully uploaded the photo to the DS-160 please bring one with you on the day of your appointment.
- A letter from the patient's doctor in Greece explaining the nature of the illness
- A letter from the patient's doctor in the U.S. stating:
- The date and time of the patient's appointment in the U.S.
- The nature of the treatment to be administered.
- The time required for the patient to remain in the U.S.
- The maximum cost of the treatment, including any likely complications.
- Proof of ability to pay for medical services in the U.S.:
- If a Greek organization will pay for the medical service, the applicant must present a letter from that organization stating that it agrees to cover the expenses.
- You will need to present proof of payment if the hospital requires prepayment of the medical expenses.
- If you cover the medical expenses yourself, you will need to present your latest original tax return statement and bank statements that prove your ability to cover the medical expenses and stay.
- If the medical expenses are covered by a U.S. relative of yours he will need to provide you with the following documents:
- an affidavit of support certified by a notary public,
- photocopy of the 1040 tax form and
- photocopies of bank statements.
- If you return to the United States for a follow-up, you will need to present documents proving that you have fully disbursed the medical center where you were hospitalized for your initial treatment.
Additionally, non-Greek passport holders will have to present:
- Greek residence and work permits and
- Evidence of sufficient funds to cover all expenses while in the United States; and evidence to show that the applicant has a residence abroad to which he/she intends to return at the end of the stay in the United States. Such evidence includes family, profession, property, employment or other ties and commitments to some country other than the United States that is sufficient enough to cause the applicant to return there at the conclusion of his/her stay.
Reminder: We always recommend that individuals apply for visas well in advance of their proposed date of travel. No assurances regarding the issuance of visas can be given in advance. Therefore, final travel plans or the purchase of non refundable ticket should not be made until a visa has been issued and you are in receipt of the passport.
