Damaged Passports
Passports that are damaged or in poor condition should be replaced, regardless of their remaining validity. For security reasons, damaged passports may be retained by this office. Please note that your damaged passport is not proof of citizenship or identity. You will be required to present either evidence of citizenship or appropriate identification. If you are not able to do so, please contact this office for further information.

Step 1 - Complete the form DS-11, Application for a U.S. Passport
Applicants applying in person must complete Form DS-11. Please print only the last two pages of the form on separate sheets of paper. Do NOT print this form on the front and back of the same sheet.

Step 2 - Proof of citizenship - One or more of the following:
- a U.S. Birth Certificate;
- a U.S. Consular Report of Birth Abroad;
- a U.S. Certification of Birth;
- a U.S. Certificate of Citizenship;
- a U.S. Naturalization Certificate;
- a passport of your U.S. citizen parent(s) in which you are included; or
- your original, long-form birth certificate, if your proof of citizenship is the U.S. passport of a parent in which you are included.
Step 3 – Proof of identity – one or more of the following:
- a valid passport of any country;
- a valid driver's license with photograph;
- a national identity card with photograph;
- a U.S. military I.D. card
Step 4 - Pay the applicable fee
Step 5 - Passport Photographs
Please submit two identical, recent color photos (taken within the last six months), 5cm x 5cm with a white background.
U.S. passport photographs are different from Greek passport photographs and many Greek photographers are not familiar with U.S. photo specifications. We strongly recommend that you print out the photo requirements and take them to the photographer. Photographs that do not meet the specified requirements will be rejected. Vending machine photographs and photographs printed on a home computer are unacceptable.
Step 6 - Scheduling the Appointment
You are required to apply for the passport in person through a pre-arranged appointment
